Up to each district how those two teams are identified. Some districts, like 7AAA, give an automatic berth to the region to the regular season outright champion. Some districts do not do this. Correct me if I am wrong davidlimbaugh but I do not believe you even have to have a district tournament? As you said, you are only required to send two teams to the region. TSSAA does not care how each district determines its two representatives, so yes, some districts could go IVY League and not even have a district tournament.
