I think there are a lot of factors. Population is a natural one. Popularity/emphasis of the sport, largely because of UT, also drives a lot of kids to play. Competition, coaching, and the availability of personal training have also greatly improved here.

I do think this is a misnomer in 6A, as Knox-area teams (aside from Maryville) aren't nearly as dominant.

I'd say the presence and dominance of both Alcoa and Maryville have contributed to that as well.  They raised the bar for the schools around them and in 4a/5a schools have responded.

Completely agree. Having that level of competition, along with the private schools, has really ratcheted things up.
