Calling a player out for slinging the bat is not an option for an umpire under TSSAA rule. After a team has been given a warning for a batter slinging the bat, any subsequent batter is ejected not out. It is a delayed dead ball. If the batter reaches base successfully, he is ejected, and an eligible sub is entered to pinch run in his place. The run should score, the player should have been subbed out, and the next batter placed in the box. Now the situation is one out with a runner on second. Although the call didn't "technically" affect the outcome, the ump still missed it. If you'll bear with me, I've always had a problem with people saying a call didn't affect an outcome; hence, I use "technically." Almost every call does. In this case, the batter returned to bat again. If the rule is applied correctly, he's done for the game. In addition, one blown safe/out call "alters" who should have actually batted in a later situation in subsequent innings. Picky but true.
